If you have a PESEL number, you should be able to do it yourself thru IKP (Internetowe Konto Pacjenta). In case you've not been assigned to a PESEL number, I can issue it for you during an online consult. You will need your passport ID number, date of birth, name of issuing country and your full name along with your residential address in Poland.
